The Timeline
What the First Year Actually Looks Like
This is the care schedule — who you see, what gets measured, and when. How quickly you personally notice a change depends on your labs and your history, so that is a conversation for your provider at your consult rather than something a web page should promise you.
You begin the plan your provider prescribed, and your route of administration is walked through with you directly rather than left to a video. Expect your closest contact with the clinic this month — it is when the practical questions surface, and asking costs you nothing.
Your provider orders follow-up monitoring labs and reads them against the baseline from your original panel. This is the first real checkpoint, and where your plan gets adjusted if your numbers call for it. These labs are included.
Once your plan is stable the rhythm settles into periodic monitoring labs and provider check-ins. You are not re-explaining yourself at every visit, because your provider already has your history and your trend line.
Monitoring does not quietly taper off once you feel settled. It continues for as long as you are on therapy — the practical difference between doing this in a clinic and doing it through a website that mails you a vial.
Evidence
What the Research Actually Shows
Testosterone therapy has been tested in randomized, placebo-controlled trials. Below is what those trials found — the trial, the population and the timeframe for each, so you can see where the numbers come from.
Sexual Function
The Testosterone Trials (New England Journal of Medicine, 2016) randomized 790 men aged 65 and over with testosterone below 275 ng/dL. Sexual activity, sexual desire and erectile function all improved significantly versus placebo over one year.
Mood
In the same trials, men receiving testosterone reported modestly improved mood and lower depressive symptom scores than men receiving placebo.
Body Composition
Pooled trial data in middle-aged men found roughly 1.6 kg less total body fat and roughly 1.6 kg more fat-free mass with testosterone versus placebo (Isidori et al., Clinical Endocrinology, 2005).
Bone Density
A controlled trial found increased volumetric bone density and estimated bone strength after one year, most clearly in the spine.
How to read these numbers
These are group averages from trial populations — largely men aged 65 and over with clinically low testosterone — and may not reflect what any individual experiences, including men in their thirties, forties and fifties. They describe what happened in a study, not what will happen to you. Testosterone therapy is prescribed only when clinically appropriate after evaluation by a licensed provider. Individual results vary; no outcome is guaranteed.

A 3-month prepay is $429 (about $143/mo) and adds one free month of recovery access.
Do I need labs before starting?
Yes. TRT is never started on symptoms alone. Your provider orders the $149 diagnostic panel first and reviews the results with you before any treatment decision.
Are follow-up labs included?
Yes. Every routine follow-up monitoring lab is included in the $159/mo membership for active, eligible TRT patients — no per-draw charges. Initial diagnostic labs are separate.
How is TRT administered?
Most patients use weekly injections; topical options exist too. Your provider recommends a route based on your labs, history and preferences.
How long until I feel a difference?
Many men notice energy and mood changes within the first several weeks; strength and body-composition changes build over months. Timelines vary and results are not guaranteed.
Can TRT affect fertility?
Yes — testosterone therapy can suppress sperm production. If fertility matters to you, tell your provider before starting so your plan can account for it.
Already on TRT — can I transfer?
Yes. Bring labs from the last 90 days and you skip the $149 panel; your first month is $99. Your provider reviews your current protocol before continuing or adjusting it.
Do I have to stay on TRT forever?
Honest answer: for most men, yes — TRT is ongoing. Starting treatment doesn't restart your own production, so if you stop, levels return to where they were and the symptoms usually come back with them. Some men do stop, for personal or clinical reasons, and your provider will taper you down rather than have you quit abruptly. Go in knowing this is a long-term decision, not a short course.
Does insurance cover TRT?
We're cash-pay, which keeps it simple. $159/month covers your treatment and follow-up monitoring labs — no claims, no prior authorizations, no surprise bills months later. Some patients submit receipts to an HSA or FSA; check with your plan. The $149 initial diagnostic panel is separate and one-time.
Is TRT safe? What about side effects?
Like any prescription therapy, TRT carries potential side effects and isn't right for everyone. That's what the lab panel and consultation are for — your provider reviews your bloodwork, health history and goals before anything is prescribed, and monitoring labs continue throughout treatment.
