Persistent fatigue — the kind that survives a full night of sleep, a vacation and a second cup of coffee — is one of the more reliable early indicators that something measurable has shifted in your physiology.

The difficulty is that fatigue is nonspecific. Your thyroid, your sex hormones, your iron stores, your blood sugar and your sleep architecture can all produce the same three complaints: exhausted, foggy, and heavier than you were last year. Telling them apart is not guesswork. It is a fairly short list of blood tests and a careful history.

This guide walks through what a thorough fatigue evaluation actually examines, which symptom patterns tend to point where, and how to prepare so your results are worth reading. It is educational rather than diagnostic — but it should make your next conversation with a provider considerably more productive.

Why "I'm just tired" is worth investigating

Tiredness becomes clinically interesting when it is persistent, disproportionate, or new. A hard training block or a newborn explains exhaustion. Six months of feeling flattened by a normal Tuesday does not.

Three questions help sort this out before any blood is drawn:

1. Your thyroid

Reverse T3 is widely marketed and rarely useful — the ATA states that in healthy, non-hospitalized people, measuring it does not help determine whether hypothyroidism exists. Free T3 is often unreliable, and T3 is typically the last test to become abnormal in hypothyroidism. 2. Low testosterone in men

The American Urological Association's guideline is specific: a diagnosis of testosterone deficiency requires two total testosterone measurements drawn early in the morning, both consistently below 300 ng/dL, interpreted alongside actual symptoms and signs. A single afternoon draw is not a diagnosis. Neither is a low number in someone with no symptoms.

This is the cause most often missed, because many people are never tested past a standard CBC. You can have depleted iron stores and a perfectly normal hemoglobin. Iron deficiency without anemia is increasingly recognized as its own cause of fatigue, poor exercise tolerance and impaired concentration, and research in non-anemic women with low ferritin has found that correcting iron status meaningfully reduces fatigue.

Risk is higher with heavy menstrual bleeding, pregnancy, endurance training, plant-based eating, bariatric surgery, or any source of slow gastrointestinal blood loss. Ferritin is the key test; conventional thresholds for iron deficiency without anemia sit below roughly 30 ng/mL, with some clinicians using a higher cutoff when symptoms are present. Iron studies and a CBC round out the picture. Iron should not be supplemented indefinitely without knowing your numbers.

5. Vitamin D and vitamin B12

Neither is a glamorous answer, and both are common. Can a normal TSH still miss a thyroid problem?

TSH is the best single starting test, but it is not the whole picture. Free T4 clarifies how the gland is actually functioning, and thyroid antibodies can identify autoimmune thyroid disease before function changes. Pituitary causes of hypothyroidism can also produce a low or normal TSH with a low free T4. If symptoms are strong and TSH is normal, the conversation should continue rather than stop.

Should I just take iron, vitamin D and B12 and see if I feel better?

Testing first is the better sequence. Iron in particular should not be taken indefinitely without knowing your ferritin, since excess iron carries its own risks.
