
Fatigue is the symptom people apologize for. It gets mentioned at the end of a visit, half as a joke, as though being tired all the time were a character flaw rather than a signal. It isn't. Persistent fatigue — the kind that survives a full night of sleep, a vacation and a second cup of coffee — is one of the more reliable early indicators that something measurable has shifted in your physiology.
The difficulty is that fatigue is nonspecific. Your thyroid, your sex hormones, your iron stores, your blood sugar and your sleep architecture can all produce the same three complaints: exhausted, foggy, and heavier than you were last year. Telling them apart is not guesswork. It is a fairly short list of blood tests and a careful history.
This guide walks through what a thorough fatigue evaluation actually examines, which symptom patterns tend to point where, and how to prepare so your results are worth reading. It is educational rather than diagnostic — but it should make your next conversation with a provider considerably more productive.
Tiredness becomes clinically interesting when it is persistent, disproportionate, or new. A hard training block or a newborn explains exhaustion. Six months of feeling flattened by a normal Tuesday does not.
Three questions help sort this out before any blood is drawn:
Hold those answers. They determine which of the causes below deserves attention first.
The thyroid is the first suspect for good reason. Analyses combining national survey and insurance claims data have estimated overall hypothyroidism prevalence — overt plus subclinical — at roughly one in ten US adults, and it is considerably more common in women and with increasing age.
The classic pattern is fatigue plus cold intolerance, dry skin, constipation, hair thinning, a slower heart rate and modest weight gain that doesn't track with your eating. Note the word modest: the American Thyroid Association is explicit that untreated hypothyroidism typically accounts for a relatively small amount of weight, much of it fluid rather than fat.
What gets tested. The ATA describes TSH as the best initial test of thyroid function, because pituitary TSH output changes before circulating thyroid hormone levels drift out of range. A high TSH with a low free T4 indicates primary hypothyroidism. Thyroid peroxidase and thyroglobulin antibodies help identify Hashimoto's thyroiditis as the underlying cause.
What usually doesn't help. Reverse T3 is widely marketed and rarely useful — the ATA states that in healthy, non-hospitalized people, measuring it does not help determine whether hypothyroidism exists. Free T3 is often unreliable, and T3 is typically the last test to become abnormal in hypothyroidism. A panel that leads with reverse T3 is usually selling a narrative rather than answering a question.
Fatigue, reduced motivation, lower libido, poorer recovery from training and creeping central weight gain is a recognizable cluster in men — and it overlaps almost perfectly with "stressed and sleeping badly," which is exactly why it needs a number rather than a hunch.
The American Urological Association's guideline is specific: a diagnosis of testosterone deficiency requires two total testosterone measurements drawn early in the morning, both consistently below 300 ng/dL, interpreted alongside actual symptoms and signs. A single afternoon draw is not a diagnosis. Neither is a low number in someone with no symptoms.

For women in their forties and early fifties, fatigue frequently traces back to sleep that has quietly stopped being restorative. Vasomotor symptoms — night sweats and hot flashes — fragment sleep even when someone believes they slept through. Research in midlife women has reported insomnia in roughly four in ten during perimenopause, and US survey data has found the majority of perimenopausal women with vasomotor symptoms report sleep disruption.
Cycle changes, mood shifts, joint aches and brain fog often arrive alongside. Because hormone levels fluctuate widely during the transition, a single estradiol or FSH value is often less informative than the pattern and the timeline — which is why history matters as much as labs here. This is the cause most often missed, because many people are never tested past a standard CBC. You can have depleted iron stores and a perfectly normal hemoglobin. Iron deficiency without anemia is increasingly recognized as its own cause of fatigue, poor exercise tolerance and impaired concentration, and research in non-anemic women with low ferritin has found that correcting iron status meaningfully reduces fatigue.
Risk is higher with heavy menstrual bleeding, pregnancy, endurance training, plant-based eating, bariatric surgery, or any source of slow gastrointestinal blood loss. Ferritin is the key test; conventional thresholds for iron deficiency without anemia sit below roughly 30 ng/mL, with some clinicians using a higher cutoff when symptoms are present. Iron studies and a CBC round out the picture. Iron should not be supplemented indefinitely without knowing your numbers.
Neither is a glamorous answer, and both are common. Analyses of national survey data have estimated vitamin D deficiency in roughly a quarter to a third of US adults, with insufficiency in a further large share. Vitamin B12 deficiency produces fatigue, cognitive changes and — when prolonged — nerve symptoms such as numbness or tingling.
B12 deserves particular attention if you take metformin long term, which has been shown in studies to lower serum B12, or if you follow a vegan diet, have had gastric surgery, or use acid-suppressing medication chronically. Both are inexpensive to measure and straightforward to correct once measured. The 3 p.m. collapse, the post-meal heaviness, the mid-afternoon reach for sugar — these are worth quantifying rather than describing. The American Diabetes Association's 2026 Standards of Care define prediabetes as an A1C of 5.7–6.4%, a fasting plasma glucose of 100–125 mg/dL, or a two-hour value of 140–199 mg/dL on a 75 g oral glucose tolerance test.
Prediabetes frequently produces no symptoms at all, which is precisely the argument for testing rather than waiting. When metabolic dysfunction is part of the fatigue picture, addressing it tends to improve energy alongside every other downstream marker. If you wake unrefreshed after seven or eight hours, snore, have been told you stop breathing, wake with headaches or a dry mouth, or fall asleep easily during the day, obstructive sleep apnea belongs near the top of your list. Recent modeling of US adults suggests the large majority of people with OSA — on the order of 80% — have never been diagnosed.
No hormone panel will fix untreated apnea, and testosterone, thyroid and metabolic numbers can all look unhelpful while sleep-disordered breathing goes unaddressed. Sleep evaluation is often the highest-yield step in a fatigue workup, and it is the one most frequently skipped.

Sometimes everything lands in range and you are still exhausted. That result is not a dead end, and it is not a verdict on whether your fatigue is real.
Reasonable next considerations include a formal sleep evaluation, screening for depression and anxiety — both of which cause genuine physical fatigue — reviewing medication side effects, assessing chronic under-eating or under-recovery in people who train hard, and looking at markers that sit outside a basic panel. Repeating a borderline value after an interval is also legitimate, since single-point results have more variability than most people assume.
Recovery habits matter alongside all of this. Consistent sleep timing, adequate protein, resistance training to defend lean mass, and deliberate downshifting are not substitutes for a diagnosis, but they are the substrate everything else works on. TSH is the best single starting test, but it is not the whole picture. Free T4 clarifies how the gland is actually functioning, and thyroid antibodies can identify autoimmune thyroid disease before function changes. Pituitary causes of hypothyroidism can also produce a low or normal TSH with a low free T4. If symptoms are strong and TSH is normal, the conversation should continue rather than stop.
Testing first is the better sequence. Iron in particular should not be taken indefinitely without knowing your ferritin, since excess iron carries its own risks. Measuring also tells you whether an abnormal result has an underlying cause worth investigating — low B12 or low iron is sometimes a clue rather than the whole answer.
For most people, no. The American Thyroid Association's position is that in healthy, non-hospitalized individuals, measuring reverse T3 does not help establish whether hypothyroidism exists and is not clinically useful.
It depends entirely on the cause, the individual and the treatment, and no honest answer is a fixed number of weeks. What is reasonable is agreeing in advance on which symptoms and which markers you will re-check, and on what timeline — so improvement is something you can observe rather than something you have to feel your way toward.
